Instructoins:
1.
List each CVPPA member that played on your team during the year. Substitutes
are not eligible to receive prize fund monies per our by-laws.
2. Determine how
much of your teams total prize fund, either by simple % or hard $$ amount and enter in second column.
3.
Make any comments necessary to justify an unusual amount for individual Player
4.
Captain needs to sign form
5. Put in envelope
with score sheet and money.
6. If further explanations are required, write
on back of this form.
7.
The League’s Executive Officers will review each form and if necessary contact Team Captain
for any further info.
8.
Allocated monies will be distributed in CASH Envelope to each Player at Banquet, or by CHECK
and mailed if requested.
9. No remittance of funds will be distributed
to Teams without completion of this form.
